Your First Visit, Step by Step

1. Get in Touch

Call 770-591-7929 or fill out our contact form. Your message goes straight to Leah, our New Patient Coordinator. She’ll follow up personally, answer any questions you have, and send your welcome materials — including your new patient forms and a short welcome video from Dr. Morgan.

2. Arrive and Relax

When you walk in, Leah will meet you in the lobby personally and take you somewhere private to confirm your paperwork. Then you’ll get a full office tour — where the bathroom is, what services we offer, where you’ll be sitting — before anyone looks in your mouth.

3. Your Exam

Adult new patient appointments run 2 hours and include a full set of imaging, a gum health check, an oral cancer screening, and one-on-one time with Dr. Morgan, Dr. Posey, or Dr. Shepperd. You’ll leave understanding exactly what’s going on in your mouth and what your options are — no guessing games, no pressure.

Patient Forms

We want your visit to be smooth and efficient. That’s why we’ve made our patient forms available online. They take about 7 minutes to complete. It’s way easier to do them here than on a clipboard in the lobby when you might be feeling hurried. So, help us help you by filling them out before you arrive.

New Patient Intake Form

Your basic info, medical history, and what brings you in. Takes about 7 minutes.

Notice of Privacy Practices

Our HIPAA-compliant privacy policy. Review before your first visit.

Financial & Insurance Authorization

Confirms how we’ll handle insurance filing (if applicable) and financial responsibility.
